Job Summary:  Requirements: Bachelor degree in psychology, social work or related social science and 2 years of experience in a mental health setting OR a satisfactory equivalence of education, experience and training. Have and maintain a valid driver license, employer to approve driving record. Pass criminal background check and pre employment chemical drug screen.

Also, applications will be evaluated based on: Knowledge of social services theory, practices and procedure; human behavior as it relates to crisis situations and problems; laws relating to mental health; skill to write clear and concise correspondence, records, treatment plans, reports and statistical data; interview, counsel and listen; family and group dynamics; ability to communicate effectively both in written and oral forms; plan, consult, coordinate with physicians, source agencies, families and the community; participate in diagnostic and treatment conferences; work with and relate to a diverse population of people with mental and physical disabilities; teach socialization and living skills to individuals with various learning levels; courteously meet and deal effectively with other employees, clients, families, schools, service agencies, service providers, physicians, professionals team and committee members and the public.

Job Duties: Perform skilled case management for clients with the Developmental Disabilities. Assess needs, environment, and existing treatment, including advocacy and resource referral for individuals with developmental disabilities. Organize, maintain and complete intake and referral process. Evaluate and assess client needs, choices and preferences; develop, review and revise individualized support plans; coordinate and monitor progression of plan. Assess and identify child and family medical, social and or / educational needs and coordinate service team to meet client and family needs. Provide advocacy for clients in legal system including victim assistance; provide protective services through referrals. Provide information and / or consulting to client provided in client homes, residential living and school settings. Follow up with clients, by phone or home visits regarding treatment and / or referrals; remain calm and use good judgment during confrontational or high pressure situations. Screen, monitor and maintain client waiting lists. Provide consultation and training to clients, professionals and community service agents; attend training sessions; develop, modifies and / or obtains training programs and materials. Serves as a member of inter disciplinary team to give and exchange information on clients; serve as staff representative to various committees. Promote community interest in and support of developmental disabilities programs. Organize, gather, maintain and complete required records, reports and statistical information; establish and maintain client recordkeeping; maintains statistical information for reporting purposes. Work is performed in an office environment, residential living setting, schools and in the field. Attention to personal safety when in contact with potentially aggressive clients.

Wage: $14.62 per hour plus benefit package, including retirement.
