Requirements: Graduate of an accredited nursing program. Current Registered Nurse licensure in the State of Oregon. Valid Oregon Drivers license as job requires regular travel to and from patients' homes. A minimum of two years acute clinical or home health / hospice experience.
Employer Preferences: BSN. One year home health experience. Recent critical care experience or disease management of chronic illnesses highly desired. Certification in one of the following specialty areas: Diabetic Education ( CDE ), Rehab, Cardiac Care, Psych, or Pediatrics.
Job Duties: Provide clinical nursing support to current patients and families. Respond to needs utilizing the professional health care process of assessment, problem identification / planning / intervention and clinical goal / outcome evaluation.
Hours: Per diem ( per day ); day shift; between 8:00 a.m. and 5:00 p.m.
